Case Management and Coordination
Georgia MENTOR’s Case Management and Coordination program provides service planning, implementation, and coordination for the adults and children we serve and their families. Upon admission to our programs, the individual receives a formal assessment by a trained case manager which includes a dual diagnosis assessment.
Our case manager, using the assessment, works with our clinical specialists, the home provider, the individual, and the family to develop an Individual Services Plan (ISP). The plan is specifically designed to meet the needs and goals of the individual. By personalizing each plan, we are able to support adults and children with a wide range of intellectual and developmental disabilities, mental health challenges, and complex medical conditions. The plan is assessed annually and altered based on how the individual’s needs change over time.
The Case Management & Coordination program also includes emergency and crisis support and advocacy services. Our case managers help ensure that each individual in our programs has the personalized services and supports they need to thrive in the communities they call home.
